January 1, 2008, the following policies will be in effect.

All graduate forms must be typewritten.

Forms are available on the web and can be typed directly on the form and printed.  Unfortunately, the form cannot be saved once it is completed (at this time).  After Jan. 1, we will return any form that is not typewritten.

Revised Graduate Forms 2 and 8 are now available on line at our website. Please use this web address as a link on your website.  We will have the most up-to-date forms.

Require names of committee members be typed next to or below signature.

Legible print is acceptable.

Forms I-A and VI-A – committee changes.

Need signatures from all committee members, new and outgoing.  Should also include a reason for the change(s).
